Radiocast: Keeping Track of Your Health Information

Rand: This is Healthcare 411: Information for better health. Personal health records: AHRQ Director Dr. Carolyn Clancy explains why they’re important and what information to include.

Dr. Clancy: Keeping track of your medical information makes you an active partner in your health care and allows you to ask good questions about your care. There are both low-tech and high-tech solutions for creating personal health records. The low-tech solution involves organizing your paper medical records and storing them in one place. The high-tech solution is an electronic personal health record. With either the low or high-tech option, you should make sure your personal health record gives a complete summary of your health history. You’ll want to list all the medicines you take, as well as how much and how often you take them. Be sure to provide information on allergies, immunizations, and test results. Giving doctors accurate information helps them provide better care.

Rand: Healthcare 411 is produced by AHRQ, the Agency for Healthcare Research and Quality, part of the U.S. Department of Health and Human Services. I’m Rand Gardner for Healthcare 411.
